Therapist, Assertive Community Treatment

The ACT Clinician/Case Manager will provide assertive community treatment to assigned caseload to avoid inpatient level of care or to assist with transition from hospital to community. Serve as part of the inter-disciplinary team to meet contractual performance indicators with OBH for this evidence based practice. Collaborate with the ACT team to meet fidelity of the CO-ACT model.
 
Essential Duties:
  • Provide Assertive Community Treatment to assigned caseload including service planning, monitoring, follow-up, linkage and crisis management for consumers
  • Provide services to meet need: in community, office-based and in consumer home
  • Make appropriate referrals to and act as liaison with community agencies, service providers and natural support systems
  • Engage consumers and appropriate family members/social supports in strengths-based treatment to stabilize and/or prevent crisis situations
  • Meet required performance standard of completing paperwork on time 95% of the time (CCAR, Treatment Plan, Case Closing)
  • Submit 90% of all PTD forms within three working days
  • Meet required number of Consumer Service Hours as determined by Network Director each month
  • Adhere to CO-ACT Fidelity Model to ensure compliance with the program
  • Provide assessment of ADL (activities of daily living) skills
  • Provide individual, group and case management services as indicated and identified in the consumer treatment plan
  • Maintain communication links with other Jefferson Center programs (i.e.: Adult Outpatient, Access/Emergency, Inpatient Liaisons, Residential, Navigation, etc.)
  • Ongoing monitoring of assigned caseload, assessment of symptoms to prevent relapse and follow up with appropriate services as needed
  • Assist with monthly data tracking and reporting for OBH
  • Participate in other EBPs (DBT, IDDT) as assigned by Network Director
  • Participate in supervision by coming prepared with an agenda; report high risk/problem cases, and utilize a problem solving approach
  • Participate in daily ACT team meetings to review schedule and coordinate outreach/activities with the team
  • Attend supervision at times and intervals agreed upon with supervisor
  • Ensure that fee collections meet the team goal as determined by Network Director
  • Exhibit enthusiasm, respect, adaptability, flexibility and spirit of cooperation in the work environment; demonstrate knowledge of principles of trauma informed care
  • Maintain effective interpersonal relations with consumers, peers, subordinates, upper management, visitors and the general public
  • Effectively respond to phone call and other requests in timely manner
  • Effectively respond to consumer needs and problems, initiate and maintain positive interaction
  • Demonstrate knowledge and skills to develop a therapeutic alliance with consumers and work effectively with consumers from diverse backgrounds
  • Work cooperatively with other community agencies, as appropriate, and in agreement with supervisor
Other Duties:
  • Attend mandatory in-services
  • Participate on Center work groups/task forces as assigned
  • Use language and behavior to promote dignity/respect with consumers, staff and external customers
  • Continually strive for self-improvement to learn new skills to benefit consumers and Center
  • Demonstrate cultural competency, awareness, and skills in work
  • Other duties as assigned

Education, Knowledge, Skills & Experience Required:
  • Master’s degree (from accredited University/College) in Counseling, Psychology, Social Work or related field required. 
  • Two-years relevant experience required. 
  • Clinical Licensure (LCSW, LPC, LMFT) preferred. 
  • CAS/CAT preferred but not required.
  • Motor Vehicle Insurance Coverage requires applicant must be 25 years or older. Must have an acceptable Motor Vehicle Driving record as determine by our insurance carrier. An MVR will be pulled along with a background check.

  • Experience with chemical dependency and abuse

Salary Grade 51-Master’s Unlicensed Clinician 
  • $46,400 to $58,900**
  • *Salary is determined based on years of total relevant experience.
  • *Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE. 
Grade 52-Licensed Clinician (LCSW, LMFT, LPC) 
  • $55,600 to $70,900
  • *Salary is determined based on years of total relevant experience.
  • *Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE. 
**BONUS OPPORTUNITY!! Three phase bonus paid out using the following schedule. ** Based on full time (40 hours/week). Prorate if less than 40 hours/week. Must work a minimum of 20 hours/week to be eligible for bonus. Current Jefferson Center employees are not eligible for sign on bonus*
